  • Harbor Behavioral Health (HBH) is dedicated to serving a diverse range of individuals, with a special focus on adults aged 21 years and older who are living with Severe Mental Illnesses (SMI). Our services are tailored to meet the unique needs of this population, offering compassionate, evidence-based care designed to promote independent living, enhance quality of life, and improve overall well-being. We understand the complexities of SMI and strive to create a supportive environment where observable behaviors are addressed through structured programs and personalized therapeutic approaches.   • TRP: Psychiatric therapeutic rehabilitation is an aspect of treatment that focuses on helping the person return to an optimal level of functioning and to achieve their life goals. This helps the person find a meaningful role for themselves, at home and at work.

    Case Management: Case management refers to the process of planning, coordinating, and managing the various aspects of each patient’s treatment to achieve specific goals or outcomes.

    Psychotherapy: Psychotherapy, also known as talk therapy or counseling, is a therapeutic process in which a trained mental health professional helps individuals explore and understand their thoughts, feelings, and behaviors.

    Group Therapy: Group mental health therapy, often referred to as group therapy, involves a mental health professional facilitating a therapeutic session with a small group of individuals who are facing similar challenges or issues.

    Peer Support Services: Peer support services in mental health involve individuals with lived experience of mental health and addiction challenges providing support, encouragement, and assistance to others facing similar struggles.

    Primary Care: Integrated behavioral health and primary care services is a healthcare model that combines both physical health and mental health services within a single, coordinated, and comprehensive system. Individuals receive more holistic and coordinated care that considers both their medical and psychological well-being.

  • The focus and purpose of psychiatric rehab is based on your mental health, instead of your physical body. TRP addresses severe mental illnesses, including schizophrenia, bipolar disorder, and major depressive disorder. The focus is on cognitive, emotional, and social functioning.

  • Our staff will gather some information about you or the patient. We will verify your benefits to let you know what services are covered by your insurance at our facility. A clinician will then conduct an assessment with you either at your residence or at our facility to discuss recommendations and a treatment plan.

  • We encourage anyone in crisis to call or text 988. This line is available 24/7/365 and free of charge. Callers will connect with a compassionate, highly trained counselor who can help address their issues as smoothly as possible.

  • Medicaid and some commercial insurance, including Anthem, Humana, Aetna, Cigna and United Healthcare.

  • To qualify for TRP, a patient must have a severe mental illness. Our intake staff is trained to identify and discuss this with our patients.

  • Peer Support Specialist - a trained individual with personal experience in mental health recovery who offers support and guidance to others facing similar challenges.

    Case Manager - a professional who coordinates and manages the care and services for individuals with mental health needs, ensuring they receive comprehensive, personalized support.

    Family Nurse Practitioner - a highly skilled healthcare professional who provides comprehensive and personalized medical care to individuals and families, focusing on overall health and well-being.

    Licensed Clinical Social Worker - a specialized professional who provides therapy and counseling, focusing on mental health and emotional well-being, and helps individuals navigate life's challenges through evidence-based practices.

    Psychiatric-Mental Health Nurse Practitioner - a specialized nurse who provides advanced psychiatric care, including medication management and therapy, to individuals with mental health disorders, utilizing a holistic and evidence-based approach.

  • Yes! We will pick up and drop off patients within a reasonable distance. Please ask for specific details during your intake process.

  • Yes! Lunch is catered daily during the scheduled session.

  • At this time, we do not offer Telehealth Services, but we do plan to add this in the near future, both for behavioral health and medical services.

  • Our lessons are built to encourage independent living and to decrease hospitalizations and incarcerations. The lessons may include topics like life skills and activities of daily living, vocational rehabilitation, mindfulness and other coping skills, social and communication skills, self management of medications and chronic disease, and wellness/healthy lifestyle.

  • Currently, we average 10-20 patients per session. We do include some break out groups to encourage small group interaction. As we continue to grow, we will be able to offer multiple sessions throughout the day.

  • Yes, just schedule a time with one of our staff members.
